1995 powerstroke 7.7L hard to start misses and then dies vi


1995 powerstroke 7.7L hard to start misses and then dies vi, 1995 ford powerstroke 7.3 L hard to start, misses and then dies after running

This truck is hard to start unless I plug the block heater in. My main concern though is that after a few hours of running once started, the truck will begin to miss and catch itself over and over again until it finally dies or on two occasions revved up to max RPM until I cut the engine off completely.
(Even though exhaust may smell like it, starting fluid has never been used on this truck)

Parts Replaced...
Cam shaft sensor
Passenger side valve cover gasket, pigtails, and glow plugs (all tests up to Ford Spec)
Oil and oil filter
Fuel filter
Alternator
Throttle position sensor (whole unit including throttle paddle)
